Changing of an intestinal wall structure-Bioplasm 9d nls


In accordance with changing of an intestinal wall structure we have chosen some degrees of its affection, each was assigned with a certain number of points:
1st degree (intact structure of an intestinal wall) – 1-2 points;
2nd degree (changes of intestinal wall structure in a form of hyperchromogenic inclusions in a mucous layer) – 3 points;
3rd degree (changes of intestinal wall structure in a form hyperchromogenic inclusions in mucous and submucous layers, longitudinal defects in submucous layer or total affection of a mucous layer) – 4 points;
4th degree (changes of intestinal wall structure in a form hyperchromogenic inclusions in mucous and submucous layers, defects in submucous layer and total affection of a mucous layer, alternating with affection of other layers of an intestinal wall) – 5 points;
5th degree (total defeat of all layers) – 6 points.
For convenience of calculation endoscopic researches results were estimated in points taking into account severity of a disease.
So, remission was estimated at 1-2 points, aphthoid affection – 3 points, scarring – 4 points, ulcers – 5 points, ulcers-cracks – 6 points.

bioplasm machine-graphy data and results of endoscopic researches


The following data was obtained by comparison of bioplasm machine-graphy data and results of endoscopic researches.
 At endoscopic remission the structure of an intestinal wall was not changed in 93.3% of cases, affection of vessels was not detected (in 73.3% of patients) or was minor (in 26.7% of patients).

In 1 patient with remission of inflammatory process and pseudopolyposis we revealed ulcerous defects in the form of single hyperchromogenic inclusions in submucous layer, at the same time vessels affections were not expressed. It means that bioplasm machine-signs should be used only altogether for definition of an inflammatory process stage. bioplasm machine-research of this patient was carried out on next day after colonoscopy, and gas between pseudo-polyps in segmented intestine was taken for achromogenic cavities in a mucous layer. This case also says that bioplasm machine-research should be applied before colonoscopy or in 2–3 days after it.

Optimization of Bioplasm-signs calculation


During aggravation stage we noted affection of a vascular wall, which in 64.7% of cases was severe. Changes of intestinal wall structure were revealed in 82.4% of patients, that corresponded to endoscopic stage of ulcers and ulcers-cracks. However it should be noted that during aphthoid and scarrings stages (17.6% of patients), no pathologically changed vessels were observed in the presence of affected wall.
For definition of correlation degree between Bioplasm-signs (a condition of an intestinal wall, vascularisation) and stages of endoscopic changes at Crohn’s disease we used rank correlation method of Spearman. For optimization of Bioplasm-signs calculation depending on expressiveness of the process they were assigned with a certain amount of points. Undamaged intestinal wall was marked with 1-2 points.

Bioplasm nls-graphy detected inflammatory changes


As it seen from the Table 1, data of Bioplasm nls and other methods of research completely coincide at colitis, but at affection of twisted intestine only, it was possible to locate inflammatory process by Bioplasm nls-research in 29 of 31 (93.5%) cases. In 2 cases Bioplasm nls-graphy detected inflammatory changes not only in small intestine, but in large intestine also. The retrospective analysis of these cases has shown that they are caused by insufficient experience of an expert who worked at the initial stage of work. At affection of large and twisted intestine location of inflammatory process was established correctly in 38 of 40 (95.0%) cases. During analysis of false-negative results it was established that in one case failure has occurred because of shortening of the right parts of large intestine and a high arrangement of head of blind colon (changes in twisted intestine about 3 cm long were considered to be changes in large intestine). In other case Bioplasm nls-research was carried out on next day after colonoscopy that has complicated evaluation of intestinal wall structure. It means that Bioplasm nls-research must be carried out in 2–3 days after carrying out of other instrumental methods of diagnostics. Thus, we have proven high informativity of diagnostics of inflammatory process location.

Bioplasm machine-semiotics elaboration


At Bioplasm machine-research of intestines we paid attention to chromogeneity of an intestinal wall, its structure, distribution of layers, extent of inflammatory process.
The basis of large intestine inflammatory diseases Bioplasm machine-semiotics elaboration was five-layer character of intestine walls.

At the first stage of work we studied Bioplasm machine-semiotics of Crohn’s disease. To carry out this objective we studied 29 postoperative macropreparations with Crohn’s disease after various operations.

According to the objective we have surveyed 114 patients with Crohn’s disease of 16 – 73 years old (average age 33 years), among them 53 men and 61 woman. Crohn’s disease proceeded in form of ileitis in 31 case, ileocolitis – in 40, colitis – in 43. Due to inefficiency of conservative therapy and development of complications 61 patients were operated. Endoscopic research was applied in 93 cases, radiological – in 76. Due to severe pain syndrome, full scale colonoscopy was not possible in 12.9% of cases.

Bioplasm nls software-diagnostics method

To identify Crohn’s disease degree of activity a Best’s index is used. Except Best’s index there are also conditional biological markers of expressiveness of inflammatory process, such as C-reactive protein, erythrocyte sedimentation rate, number of platelets, fibrinogens, and also endoscopic and contrast methods of research. However instrumental methods of investigation have the lacks related to a risk of disease aggravation during preparation for given inspection and an irradiation. Disappearance of abscess formation sites in all patients was noted in 6 weeks after initiation of treatment, at the same time in structure of 2 sites we noted formation of small fibrous sites.

Bioplasm 9d nls analyzer-angiography has shown positive changes


Dynamic observation during a small period of time has shown that tumor sites and cystic degeneration areas do not change in structure and size; abscess formation sites, in case of a positive response to therapy decrease in size, when they disappear formation of small fibrous sites in a projection of these sites is possible.
In our research within the first two weeks (10-14 days) in 9 patients we marked reduction of abscess formation sites after applied therapy, Bioplasm 9d nls analyzer-angiography has shown positive changes in a vascular wall condition in 9 patients. In 2 cases because of positive dynamics absence, a correction of antibacterial therapy was done.

Differential diagnostics of an abscess at Bioplasm 9d nls


Differential diagnostics of an abscess at Bioplasm 9d nls is carried out with cysts and with rostate gland cancer sites. Simple cysts and cystic degeneration of prostate gland are sonographically similar to developed prostate abscess, but differ in location (more often: a single central location at congenital cysts or location in the central part against the background of adenomatously changed parenchyma). It is difficult to differentiate an abscess and cancer of prostate gland. Distinctive criteria at Bioplasm 9d nls are considered to be: location – an abscess is more often located in a transitive zone of a gland, but cancer – in peripheral; size – more often an abscess is larger than a tumor, at an abscess – presence of changes in structure and size at dynamic treatment. In diagnostically difficult cases SEA of prostate gland is recommended. In our opinion, location and size are not the exact diagnostic criteria: in the presence of large tumors a cystic component may be present in their structure, which is a site of disintegration. In 8 patients of our research, abscess formation sites were located in a peripheral zone, and the average size of them was small – comparable with a tumor site.

Bioplasm with NLS-ultramicroangiography


In 2 patients due to absence of positive dynamics according to results of SEA at Bioplasm with NLS-ultramicroangiography, correction of antibacterial therapy was fulfilled. At the control researches, carried out within 3-5 weeks during the course of treatment, positive dynamics in the form of normalisation of SEA results was detected in all patients. There was a gradual decrease of chromogeneity of the affected areas. The quantity of hyperchromogenic areas has decreased – to 5 (in 2 patients multiple abscess of the gland were diagnosed), their chromogeneity was lower in comparison with the previous researches. In patients with remaining abscess formation areas, complete disappearance of such areas happened in 6 weeks from the start of treatment. Research in a Bioplasm-angiography mode has shown in most cases gradual restoration of a vascular channel in a projection hyperchromogenic zones after treatment.

Bioplasm machine have more features and databases


On the basis of digital rectal examination prostate abscess was suspected in 4 patients, we detected: enlargement and morbidity of prostate glands, fluctuation symptom was marked. In other patients the gland was enlarged and was painful, in these cases a preliminary diagnosis was made: an aggravation of a chronic prostatitis.

Three-dimensional Bioplasm 9d nls analyzer has proved itself as simple, noninvasive and highly informative diagnostics method, thanks to absence of beam loading. The issue of Bioplasm 9d nls analyzer  application in monitoring of conservative treatment of prostate abscess is studied insufficiently.

We must emphasize that area of clinical application of bioplasm machine is being extended, because the method provides anatomical reconstruction of researched area (organ, structure), acquiring of most accurate results at evaluation of organ’s condition, analysis of ultrathin crosscuts, visualization of which is impossible at routine scanning. The information saved entirely, so it is possible to reconstruct it many times without any time limitations and in absence of a patient (patient feels no discomfort and inconvenience).

Bioplasm nls at evaluation of esophagus cancer

Bioplasm nls made technically possible application of NLS-graphy in diagnostics of esophagus affections, extending possibilities of the method in identification of a cancer stage. 3D reconstruction contributes greatly into interpretation and quantitative analysis of a situation requiring introduction of a stent at esophagus cancer. J.Lin and D. Goldberg in a preliminary study have demonstrated a possibility of 3D reconstruction of data acquired by application of NLS-ultramicroscanning; at the same time Bioplasm nls-ultramicroscanning turned out to be more precise, than spiral CT at evaluation of esophagus cancer.
Bioplasm nls-graphy of a stomach with SEA allowed us to diagnose malignant ulcer in greater curvature of the stomach and prove involvement of regional lymph nodes.

It is known that both own and transplanted liver tissue can regenerate, that is why visual methods gradually becoming a part of this process monitoring. Although today MRI and volumetric CT are used in this case more frequently, Bioplasm machine-graphy has great prospects, allowing to replace in the future these very expensive, complex and radiation related (CT) methods. Extracapsular growth of metastases in lymph nodes often leads to merging of several affected lymph nodes into a conglomeration and involvement of surrounding tissues into tumorous process. Involvement of adjoining organs into a tumorous conglomeration must be identified by SEA of their morphological structure.

At malignant processes in lymph nodes, central achromogenic area is visualized most frequently, which corresponds to diffuse or total replacement of central areas by necrotic tumorous tissue. In certain cases at metastatic affection of a central part of a lymph node only, it may look unchanged, because an imitation of healthy lymphoid tissue happens (it is often detected at well differentiated squamous cell cancer and other types of cancer, which are accompanied by coagulation necrosis in case of ischemic degeneration). Sometimes a lymph node may be represented as totally achromogenic formation. Although the latter is true for fatty infiltration, in these cases tumorous genesis of changes cannot be excluded completely. In this case only SEA will help to set a diagnosis.

In normal state cortical substance is represented by slightly chromogenic (almost achromogenic – 1-2 points at Fleindler’s scale) homogeneous frame. Central parts (trabecules, parachylarious thickening, fatty inclusions, partially – medullary substance) are represented by hypochromogenic structure (2-3 points), portal – by homogeneous hypochromogenic structure of irregular (triangular) shape.
Structure of lymph nodes changes depending on their location. So, at analysis of structure of cervical lymph nodes located close to an organ, we see that the closes to an organ lymph nodes (non-permanent) are of smaller size, have roundish or spindled shape, and lesser presence of cortical substance. In lymph nodes located far from an organ cortical substance is developed better.

Size of lymph nodes depends on age of a studied person, peculiarities of his physique, etc. In clinically healthy people lymph nodes up to 3.5 cm may be found. There is a difference in size and shape of lymph nodes of various regional groups and lymph nodes located in various areas of one and the same person.
Shape of lymph nodes generally depends on their topographoanatomical relations with surrounding organs, usually it is slightly oblong and flattened, a little prominent on the one side and minor pit on the other, resembling a bean. Sometimes shape of a lymph node may be roundish. In senile aged patients tape-like shapes of lymph nodes can be found, which may be regarded as a result of lesser nodes merging. In areas limited by fascias (for example in occipital region), nodes are more long than those located in loose tissue (for example, axillary).
