The importance of bioplasm nls health analyzer


Detection of hepatic artery stenosis in early post-transplant period promoted choosing of further treatment tactics (prescription of heparin to prevent hepatic artery thrombosis development).
Revealing of various forms of portal anastomosis (thrombosis of portal vein anterior branch – 1.4%, portal vein stenosis in area of anastomosis application – 2.9%) and inferior vena cava (inferior vena cava stenosis – 1.4%) did not require using of specifying methods of investigation.
In our studies biliary complications of relative transplantation had leading position among others, which corresponds to opinions of foreign scientists.

The main problem of late post-transplant period is affection of intrahepatic bile ducts, caused by formation of biliary anastomosis strictures. The importance of bioplasm nls in surgery


At the present moment in references there is no such concept as regional changes of transplant parenchyma chromogeneity at bioplasm nls-studies. Parenchymatous complications are divided into focal and diffuse, but we believe that introduction of concept of regional changes of parenchyma chromogeneity allows to characterize changes detected by bioplasm nls more accurately. Also there is no data regarding frequency of parenchymatous complications development.
In our monitoring diffuse increasing of liver right lobe transplant chromogeneity was registered in 5 (7.1%) patients in late post-operative period. Revealed changes were accompanied by increasing of transaminase, cholestasia enzyme and bilirubin levels and were conditioned by development of transplant dysfunction. We believe that bioplasm nls with SEA allows to identify a reason of transplant parenchyma diffuse changes development in majority of cases. Bioplasm nls with SEA data is confirmed by results of clinical and laboratory studies and materials of morphological research of liver biopsy material in 98.6% of cases.

Bioplasm nls analyzer study


Diffuse increasing of liver right lobe transplant parenchyma chromogeneity was detected only in late post-transplant period in 5 (7.1%) patients and was conditioned by development of transplant’s dysfunction.
Regional changes of liver right lobe transplant parenchyma chromogeneity in early post-transplant period (30 days) were registered in 3 (3.4%) recipients and were represented by increasing of chromogeneity in an area of VI and VII segments of liver. During late post-transplant period regional changes of liver right lobe transplant parenchyma chromogeneity were detected in another 3 (3.4%) recipients, however in this case we saw increasing of chromogeneity in an area of VI and VII segments of a transplant.
Focal changes of liver right lobe transplant parenchyma were detected in late post-transplant period in 3 (3.4%) patients. In all cases it was acute abscesses, accompanied by intensive increasing of chromogeneity (6 points at Fleindler’s scale) in a nidus.
NLS-signs of hepatic artery thrombosis were detected by bioplasm nls analyzer-ultramicroangiography in 1 (1.4%) patient during early post-operative period.
Hepatic artery stenosis in early post-operative period was diagnosed in 1 (1.4%) patient, and in another patient in late post-operative period. In both cases bioplasm nls analyzer data were confirmed by SEA results.

Non-occlusive thrombosis of portal vein in early post-operative period was diagnosed in 1 (1.4%) patient on the basis of SEA of morphological structure of thrombus in an opening of posterior branch of right portal vein, which did not close opening of this vessel completely.

Comprehensive Bioplasm nls was carried out with Bioplasm nls health analyzer with high-frequency generator of 9.6 GHz, unit of continuous spiral scanning and professional software Metapathia GR Hospital allowing to carry out three-dimensional visualization of liver.
During examination we used Bioplasm nls health analyzer-ultramicroangioscanning mode and spectral-entropic analysis (SEA), allowing to detect spectral similarity of morphological structures of hepatic parenchyma and vessels with digital models of etalon processes of healthy and pathologically changed tissues.

Bioplasm nls diagnosing lacrimal duct and lacrimal sac pathologies

Bioplasm nls with ultramicroscanning is high-efficiency method in lacrimal passages diagnostics and it allows diagnosing lacrimal duct and lacrimal sac pathologies. Multi-layer spiral CT with contrast enhancement is an informative method in lacrimal passages diagnostics and it gives the opportunity to determine exact process localization, its intensity, patency of lacrimal passages, relation with adjoining structures and its affinity for accessory sinuses of the nose.
As can be seen from the above, Bioplasm nls must be considered as 1st stage method in diagnostics of lacrimal passages diseases. In addition, multi-layer spiral CT with contrast enhancement must be considered as correction method and 1st stage method in case of nasolacrimal canal pathology.

Perform bioplasm nls after surgical intervention and placement of silicone capillary tube


Estimation of installation correctness of intubation material in lacrimal ducts was performed after surgical measures on lacrimal ducts.
Chronic post-traumatic dacryocystitis was diagnosed in 2 patients after using multi-layer spiral CT with contrast enchancement. Acute ectasia (15.5 × 7.6 mm) of lacrimal sacs on both sides with sharp and straight contours was registered. In both cases nasolacrimal ducts did not visualize due to osseous structures injury of nasolacrimal ducts.
Lacrimal ducts obliteration was diagnosed using multi-layer spiral CT with contrast enchancement. Patency blockade of lacrimal ducts with no contrast substance in subjacent parts of lacrimal passages was registered. Moderate widening of lacrimal ducts lumen up to 1.1 mm. with sharp and straight contours was also found.

In case of such pathology, it is preferable to perform bioplasm nls after surgical intervention and placement of silicone capillary tube.

bioplasm nls with ultramicroscanning, multi-layer spiral CT and contrast enhancement in lacrimal passages diseases diagnostics

28 patients with various lacrimal passages disorders (age from 18 to 80; 18 women and 10 men) were examined. After ophthalmological examination all patients underwent radiology procedure. Using bioplasm nls analyzer, all patients underwent NLS-ultramicroscanning examination. 25 patients underwent multi-layer spiral CT using bioplasm nls system with contrast enhancement by non-ionic iodine-containing medication (Omnipak 240 mgI/ml) in sinus volume scanning mode.
Contrast enhancement was performed after prewashing of lacrimal passages via lacrimal openings by physiological solution. Thickness of tomographic slice – 0.38 mm. Multiplanar reconstructions in coronary, axial and sagittal sections were modelled.

clinical-neurological diagnostics compared to bioplasm nls-research data

Clinical-neurological data was sufficient for diagnostics of ischemic or haemorrhagic stroke types (of any given localization) in 1/3 of vertebrobasilar system strokes cases (in 45 patients – 29.4%), i.e complete match of clinical diagnosis with bioplasm nls and spectral-entropy analysis data. Vertebrobasilar system stroke based on clinical-neurological data was diagnosed in 68 patients (44.4%), although type or localization within stem-cerebellar zone was not recognized, in other words, partial but quite significant difference compared to bioplasm nls data took place. Complete difference of clinical-neurological diagnostics compared to bioplasm nls-research data was found in 40 patients (26.1%), which was caused mostly by grave patient condition (unconsciousness, speech disorders, affection multiplicity, comorbidity and etc.).

Bioplasm nls diagnosis

Subacute stage of ishemic stroke (6-21 days) was characterized by demarcation increase of lesion contours. Bioplasm nls showed distinct bounded zones of infarction. Chromogeneity of these zones was still high (5-6 points according to Fleindler’s scale). Accompanied by hyperchromogeneity Bioplasm nls showed small segments of moderate chromogenic signal caused by increased protein contents.
Some necrotic patches began demonstrating clear boundaries in ischemic stroke organization stage (> 21 days) owing to edema absorption.
Demarcation due to gliosis began developing around necrosis lesion. Affection zone on Bioplasm nls usually decreased in size and gained sharp contours. About 6 weeks later necrotic masses were completely reabsorbed and replaced by glious tissue (or cyst was developing). Bioplasm nls displayed gliosis as increased chromogeneity zone, while cyst had achromogenic structure caused by liquor fluid.

Bioplasm nls in diagnostics of vertebrobasilar system strokes

Clinical-neurological and Bioplasm nls-examinations data was analyzed in 203 patients. Vertebrobasilar arterial system stroke was diagnosed in 153 patients; patients age from 23 to 85 years. Mature men is the most affected group (30.1%). Control group consisted of 50 patients with no organic changes in brain: Bioplasm nls health analyzer-data of their examination was taken as a basis for cerebellum and brain stem Bioplasm nls -anatomy study.

Clinical observation of bioplasm software


There is a clinical observation.
Patient A., 36 years. Presented problems on weight feeling in right hypochondrium, asthenia. Objectively: clear heart tones, rhythmic, pulse 86 bpm. Arterial pressure 120/80 mm Hg. Soft stomach, painless. Laboratory data: clinical blood analysis – eosinophilia, clinical urine analysis, coagulogram within normal range. Bioplasm software showed cyst formation, occupying VI–VII segments of right hepatic lobe with multilayered capsule common for echinococcus. Cystic nature of changes, localization and presence of thick capsule were specified using MRT. Echinococcus cyst in VI–VII hepatic segments was registered based on mixed diagnostics.

Calcification foci common for parasitic hepatic cysts were found in 3 patients. Cysts more than 100 mm in diameter were registered in 34 patients. Bioplasm software with spectral-entropy analysis method was the most informative and universal one, which is a screening method for patients examination with cystic lesions of the abdominal cavity. Precise diagnosis of hepatic echinococcosis was established in most of the cases when using bioplasm software. Determination of nature of intracystic inclusions (secondary cysts, septums and etc.) assisted in differential diagnostics. Serological reactions (indirect hemagglutination reaction, immunoenzymometric analysis and antigen serological diagnosis) for echinococcosis, as well as cytological, bacteriological and biochemical studies data served as spectral-entropy analysis confirmation results. bioplasm software had considerable information value in case of multiple and widespread echinococcosis. Definition of precise niveau diagnosis during bioplasm software was easily achievable in giant echinococcus cysts and multiple lesion cases. Combination of bioplasm software with CT or MRT was necessary in some cases. Complexity of differential CT-diagnostics in case of echinococcus liver diseases was associated with structural features of parasitic lesions. Differentiation between ordinary and monovesicular echinococcus thin-membrane cysts, without parasitic membranes and internal structures dissection caused some difficulties. In these cases correct diagnostics was based on mixed clinical-laboratory and radiation examinations of patients. When analyzing helical computed tomography results with bolus contrast enhancement, we have registered that contrast enhancement of surrounding liver parenchyma in parasitic cysts was diffuse-uneven in 50% of cases.

Bioplasm nls health analyzer and CT have different sensitivity and specificity in the diagnostics

It was also established that Bioplasm nls health analyzer and CT have different sensitivity and specificity in the diagnostics of complicated hepatic echinococcosis, which necessitates complex sequential application of both methods. Relatively poor indices of diagnostic efficiency of latex particle agglutination reaction do not allow using it as prehospital monotest, but the value of serological reactions increases in case of differential diagnostics of residual cavity recurrences after echinococcectomy.
MRT was performed in general group of 76 patients in the final stage of non-invasive diagnostic examination to elaborate in details the nature of pathomorphological changes in lesion focus and clarify involvement degree of major vascular secretory structures into the process.
Free choice of image planes allowed clarifying the topical diagnosis in 76 observations. On the other hand, additional significant signs of parasitic cysts in patients were detected due to the high soft tissue contrast during MRT procedure. For example, MRT was the most informative in detection of irregularities inside cyst edge and detection of parasitic membranes dissection starting process.
Thus, in 69 patients the difference of fibrous capsule hypointensity and chitin shell of the parasite showed two-layer image of cyst wall in tomogram, which was common only in parasitic cysts and it allowed its differentiating from all other cystic lesions.

Bioplasm nls in mixed diagnostics of complicated hepatic echinococcosis


Use of Bioplasm nls (which in comparison to CT provides panoramic images and higher sensitivity superior in certain locations, accurate differential and regional diagnostics) was the next step in diagnostics improvement of complicated hepatic echinococcosis. Bioplasm nls provides more efficient differentiation of «pseudo-soild» echinococcosis forms from tumors by means of spectral-entropy analysis (SEA). Overall accuracy of Bioplasm nls-diagnostics in case of echinococcosis is more than 95%.
Bioplasm nls diagnostics method has the following key advantages: effective estimation of cysts size, their number and location in liver, their interrelation with vessels and bile ducts, parasites development stage estimation and also detection of other organs affection by echinococcus. This information is crucial in surgical approach determination, planning of operation type and its scope, and also suggesting of possible intraoperative complications.
As can be seen from the above, the introduction of Bioplasm nls with SEA into clinical practice significantly improves diagnostics of complicated echinococcosis (especially in its early forms) which allows often performance of radical and, at the same time, economical intervention. However, existing methods of instrumental diagnostics do not provide 100% accurate results. In 2-9% of cases it is not possible to establish the nature of hepatic cyst formation using hardware research methods.

The purpose of given research is to determinate the peculiarities of hepatic echinococcosis hardware diagnostics and its complications.

Bioplasm nls analyzer diagnostic significance in diagnostics of knee joint osteoarthritis

During our study given disorder was found in 2 patients with II and III Rg-stages of knee-joint osteoarthritis; these patients also had evident degenerative changes of hyaline cartilage of load-carrying surface of lateral femoral condyle. Both cases confirmed by diagnostic-treatment artroscopy.
Relatively high Bioplasm nls analyzer diagnostic significance in diagnostics of knee-joint osteoarthritis having both high sensitivity/specificity of method and wide range of registered pathological changes was revealed in the course of study. Manifestations of osteoarthrosis vary and in most cases they start developing not with bone changes detected by radiography. Changes of hyaline cartilage, small osteophytes are detected during Bioplasm nls analyzer and at pre-radiological stage of knee-joint osteoarthritis, simultaneously with clinical semiotics manifestation. Many manifestations, if not reversible, but quite effectively stabilized by modern therapeutic agents at this stage of disease. This approach greatly improves the clinical significance of nonlinear diagnostics in early detection of described signs of intra-articular degeneration. When using Bioplasm nls analyzer, structural changes of hyaline cartilage are detailed visible and determination of localization of its most evident structural changes becomes possible, which often shows primary cause of knee-joint osteoarthritis development, especially in case of apparent asymmetric affection of lateral and medial parts. Certain structural changes often occur in combination that promotes some characteristic symptom groups such as: combination of kneecap lateroposition, degenerative thinning of anterior surface of lateral femoral condyle, enthesopathy and calcification of the lateral parts of quadriceps tendon. Detection and presentation of acentric osteophytosis is both important at pre-radiological stage of knee-joint osteoarthritis and at later stages, because (as our research had shown) patellofemoral arthrosis becomes predominant in early stages; standard radiographic views show such arthrosis not sufficiently clear. Minor structure anomalies of knee joint, such as kneecap lateroposition, hypertrophy of fat body and alar folds, mediopatellar fold and etc., have important practical importance since they may contribute to osteoarthritis development (especially patellofemoral one) while menisci anomalies, discoid meniscus in particular, and also their degenerative changes cause predominance of dissymmetric tibiofemoral arthrosis.

Nonlinear(Bioplasm nls health analyzer)diagnostics of early manifestations of knee-joint osteoarthrosis

About bioplasm nls-spectral entropy analysis


Examination analysis of complex patients with space-occupying lesions of salivary glands had shown that bioplasm nls -graphy is leading visualization method.
It allows clarifying size, structure, shape and tumor type using spectral-entropy analysis. Computer tomography is used in evaluation of tumors more than 3 cm. in diameter; it provides accurate estimation of volume, structure of tumor and condition of adjacent anatomical structures.

The results and discussion of Bioplasm nls


We have registered 78 pleomorphic adenomas, which comprised 54.5% of all detected space-occupying lesions of large salivary glands and 63.4% of all benign tumors of large salivary glands. In 56 (71.8%) cases pleomorphic adenomas were located in parotid salivary glands and in submandibular glands in 22 (28.2%) cases.
Bioplasm nls-graphy revealed pleomorphic adenomas of parotid salivary glands in 46 (82.2%) cases as oval or round hyperchromogenic (6 points according to Fleindler’s scale) formations with distinct contours and heterogeneous structure. Surrounding tumor of salivary gland parenchyma was not changed according to its structure and chromogeneity. Pleomorphic adenomas up to 1 cm. in diameter had homogeneous chromogenic structure, adenomas more than 1 cm. had homogeneous isochromogenic structure due to hemorrhage and discharge areas. Pleomorphic adenomas of submandibular glands were characterized by hyperchromogenic foci of various shapes.
Bioplasm nls-angiography revealed vascular bed affections in 74 (94.9%) cases of pleomorphic adenomas. Spectral-entropy analysis was used for differentiation of pleomorphic adenomas from other salivary gland benign tumors (leiomyoma, teratoma, neurinoma).
In 44 (93.6%) cases CT displayed pleomorphic tumor of parotid gland as single high-density formation (Mavrg = 29,6 ± 4,2 HU) of round-shape, with distinct edges and smooth contours. Tumor structure was heterogeneous in 25 (53.2%) and homogeneous in 22 (46.8%) cases. Average tumor size reached 2,9 ± 0,9 cm.; its sizes reached 5.1 cm. if located in deep lobe of gland. The density of unaffected part of parenchyma was 16,4 ± 5,2 HU. In 3 (3.8%) patients, however, tumor had its density equal to surrounding parenchyma, which especially complicated diagnostics of tumors located in the narrow parts of salivary glands (pole).
Unlike parotid glands formations, pleomorphic adenomas of submandibular salivary glands had no clear boundaries separating tumor from gland, tumor density corresponded to parenchyma’s, average tumor size was 3,6 ± 1,3 cm. Thus, significant increase of gland sizes compared to the opposite side and displacement of surrounding soft tissues was registered.
When using Bioplasm nls-method, most researchers describe pleomorphic adenomas of greater salivary glands as hyperchromogenic (5-6 according to Fleindler’s scale) round-shape formations.
Tumor-surrounding parenchyma of salivary gland was structually and chromogeneically not changed. According to our data, pleomorphic adenomas of submandibular glands (unlike parotid glands tumors), were characterized by isochromogenic foci of various high-chromogeneity forms. Preserved parenchyma was seen at Bioplasm nls-graphy more clearly compared to CT picture.

In 75% cases of using CT, pleomorphic adenomas had homogeneous pattern; clear, well-determined edges were found in 90% of pleomorphic adenomas, which corresponds to literature data.

143 patients with greater salivary gland tumors were examined, all of them underwent Bioplasm nls analyzer-graphy, 64 patients underwent non-contrast computed tomography (CT). 78 pleomorphic adenomas were registered in total. In 56 (71.8%) cases, adenomas were located in parotid salivary glands, in 22 (28.2%) cases adenomas were found in submandibular glands. In 46 (82.2%) cases, Bioplasm nls analyzer-graphy presented pleomorphic adenomas of parotid salivary gland as oval or round hyperchromogenic formations with distinct contours and heterogeneous structure. Vascular bed affection was registered in 74 (94.9%) of pleomorphic adenomas cases according to Bioplasm nls analyzer-angiography. Computed tomography presented pleomorphic tumor of parotid salivary glands as high-density single formation (Mavrg = 29,6 ± 4,2 HU) with round shapes, distinct lines and even contours in 44 (93.6%) cases.
Bioplasm nls analyzer-graphy demonstrated 100% of sensitivity, 96.3% of specificity and 97.7% of accuracy in detection of pleomorphic adenomas of large salivary glands; CT demonstrated 97.6, 96.4, 97.6% respectively in doing so. bioplasm nls analyzer-graphy is the main imaging method.

Bioplasm nls health analyzer Conclusion

Performed research showed that Bioplasm nls health analyzer of lumbar spine in early, intermediate and late postoperative periods had revealed changes common for all types of surgeries: scarring changes along surgical canal, recurrence or residual hernias of lumbar spine and epidural scarring. Hypertrophy of posterior longitudinal and yellow ligaments, osteoarthritis of intervertebral joints and intervertebral foramen stenosis appeared in late postoperative period.
Some operations (laminectomy, hemilaminectomy) are characterized by vertebral arch defect. Material and methods of Bioplasm nls


NLS-study results of 42 patients operated for lumbar hernias were analyzed. Bioplasm nls was performed in the period from 14 days to 16 years after surgery. Research was conducted due to recurring pain syndrome and occurrence of neurological symptoms in patients. Patients age ranged from 30 to 70 years; 27 women and 15 men. Patients underwent various operations: interlaminectomy (15) hemilaminectomy (12), laminectomy (6) interlaminar hernia excision (6), anterior interbody fusion by titanium implant with diskal hernia removal (3) at single (36), two or more (6) levels.
NLS was performed using bioplasm nls system. «bioplasm nls Hospital» special-purpose software with three-dimensional visualization of surgical contact area was used. Maximum possible reduction of total research time for low tolerance adaptation of spinal patients with acute pain syndrome for long-term lying on their backs was practiced; by doing so we have received maximum complete and accurate information on nature and prevalence of pathological process with minimal loss of image quality.

Multi-positional research conducted in 3D-mode with ultramicroscanning thus provides full information on nature of changes in all anatomical and histological structures of discovertebral unit in patients with compression nerve-root pain syndromes.

Introduction about Bioplasm nls health analyzer

From histological point of view skin consists of two layers: epidermis and derma which are closely connected. Epidermis consists of several layers: basal, spinous, granular and horny. In epidermis of palms and feet, between granular and horny layers there is additional layer – stratum lucidum. Sometimes the aggregate of basal, spinous and granular layers is called a malpighian layer.
Derma is a connective basis of skin, where perspiratory and sebaceous glands, blood and lymph vessels, nerves and smooth muscles are located. There are papillary and reticular layers of derma. Papillary layer, located under epidermis, and a part of reticular layer conditionally form upper derma; layer of derma located at the level of pilosebaceous unit is indicated as middle derma; the underlying layer containing perspiratory glands, bordering subcutaneous fat, is called a deep derma.
Blood vessels of skin form two anatomic plexuses: a deep one, located next to subcutaneous fat, and a superficial one, located in sub-papillary layer. They consist of microcirculatory bloodstream vessels: arterioles, venules and capillary tubes.
Total thickness of skin without subcutaneous fat varies from bits of a millimeter to 4 mm.
